How to build a form, and read the answers.
Open Forms, and press New form.
Add your fields. Text, email, phone, a date, a dropdown, a tick box. Mark the ones that are required, and keep it short.
Put in an address, and every response is emailed to you as it arrives.
Read the answers on the Responses tab. And export them all as a spreadsheet.
